Privacy Notice
This notice covers our website only. It does not cover the CareBridge SSM application at cm.carebridge.cloud. Information handled inside the application on behalf of a customer organization is governed by the Business Associate Agreement and service terms executed with that organization, not by this notice.
Do not send us client information. Our forms are not a secure channel for protected health information, and we ask you not to include client details in any message to us.
What we collect
If you complete the walkthrough request form, we collect only what you type into it:
- Your name
- Your work email address
- Your organization
- Your team size
The form contains a hidden field used to detect automated submissions. If it is filled in, the submission is discarded.
We do not use advertising trackers, and the site sets no cookies. Our web server keeps standard request logs, which include IP addresses, for security and troubleshooting.
Why we use it
Only to arrange and follow up on your walkthrough, and to answer questions you have asked us about CareBridge. We do not sell your information, and we do not share it with third parties for their own marketing.
Where it goes
Form submissions are delivered to CareBridge through an automation service we operate. The details are stored in our own systems and are accessible only to CareBridge staff who need them to respond to you.
How long we keep it
We keep enquiry details for up to 24 months from your last contact with us, then delete them. If you become a customer, your information is retained under your service agreement instead.
Your choices
You can ask us to show you what we hold about you, correct it, or delete it. Email legal@carebridge.cloud and we will respond within 30 days. You can opt out of follow-up at any time by replying to any message from us or emailing the same address.
Depending on where you live you may have additional rights under laws such as the GDPR or the CCPA. Contact us at the address above and we will honour any right that applies to you.
Security
The site is served over HTTPS. Enquiry data is limited to the four fields above by design — there is no field on this website capable of accepting client records.
Children
This website is intended for professional use and is not directed at children.
Changes
If we change this notice we will update the date at the top. Material changes will be noted here.
